Applicable Payments and co-payments are due at time of service.

Cash, checks and MasterCard, Visa, and Discover cards are accepted.

Insurance: I am a preferred provider with the following insurance companies:

Some out-of-network provider benefits may be accepted.

Office Hours

I have some evening appointments available for clients who work during the day. Days that I am available are subject to change.

Completing Forms

Prior to our first appointment you will need to complete Intake and Informed Consent forms. I will not be able to meet with you until the forms are completed. The forms should take about 20 minutes to fill out.

There are two ways to get the forms:

  • You can download forms from this site and fill them out and either print and mail them, or upload them to my portal. I will send you a link to the portal when we have scheduled a session. Click here to go to the forms page.
  • If you do not have the capacity to download or print forms you may request a mailed copy, file them out, and send them to my office address.

What to expect at the first session

We will talk about your concerns and how long you have been experiencing these problems. We will also discuss your personal, family, and work relationships, with a focus on: recent changes, expectations you have in these relationships, and how you are functioning in these relationships. This information will help establish a framework and direction for our work together. I may also provide information or make suggestions regarding additional treatments that might be helpful, such as couples therapy, medication management, group therapy, or social support groups.
